Registered Nurse, Care Coordinator – Plastic Surgery

Responsibilities

• Collaborates with clinic team members to provide evidence-based patient-centered care
• Helps patients stay up to date on their care plans and related goals towards improving health
• Focuses on high-risk medically complex patients and those in care transitions
• Acts as a liaison with patient’s family members and other agencies
• Facilitates improved communication with the aim of providing comprehensive whole person care
• Stays up to date on standards, assisting with programs and processes

• Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
• Current Ohio Registered Nurse license
• CPR/BLS certified
• Knowledge of triage procedures
• Clinic, physician office and/or urgent care experience
• Ability to maintain quality, safety, and/or infection control standards
• Knowledge of appropriate procedures and standards for the administration of medications and patient care aids
• Ability to educate and clearly communicate with patients and/or families as to the nature of disease
• High level of interpersonal skills to handle sensitive and confidential situations
• Analytical ability and critical thinking
