RN Home Hospice Case Manager
Location:
Columbus, OH
Shift: Day
Time:
Monday-Friday 08:00 AM to 5 PM
Duration: 12 weeks Desired
Start Date:
ASAP Dress Code:
Navy blue scrubs
- Rotating weekends and every other holiday.
- Rotate call with staff.
- Home Hospice experience required.
- 2 years of experience.
- Must hold BLS certification
- Functions of this position:
Identifying and case manages patient/family care needs to provide palliative and supportive care to terminally ill and their families. - Completes assessment of patient and family, including physical, emotional, social and spiritual factors. Responsible for direct nursing care to patients on intermittent basis in all settings of hospice care to achieve highest level of comfort and quality of life.
- Titrates analgesics for optimum pain control within parameters determined by physician.
- Teaches and counsels family and patient regarding nursing techniques.
- Provides emotional support to patients and families.
- Affirms patient/family's lifestyle and involves them in decision-making regarding care.
- Navy blue scrubs and EPIC EMR. Caseload is 15-17.
