Gentiva is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to provide hands-on care in patients' homes and facility settings. You will support the interdisciplinary team to implement individualized plans of care and report changes promptly to the RN and care team.
Responsibilities include bedside nursing under RN supervision, medication administration, infection control, and on-call availability. CPR and valid driver’s license required; 2+ years hospice/home health preferred.
