Full-Time
- Monthly base stipend of $3,500
- Additional productivity-based compensation
- Total annual compensation potential: $118,000–$200,000+, depending on productivity
The Regional Post-Acute APP serves as a highly skilled clinical resource supporting Med One partner facilities throughout Central Ohio. This role ensures continuity of exceptional patient care by providing coverage during provider vacations, leaves of absence, onboarding of new facilities, open territories, and periods of increased clinical demand.
The Regional Post-Acute APP practices across multiple post-acute care settings, collaborating closely with physicians and interdisciplinary teams while adapting to diverse clinical environments, workflows, and electronic medical record systems.
Travel is generally limited to facilities located within approximately a 60-minute one-way drive from the clinician's designated home base, with mileage reimbursement provided.
In addition to direct patient care, this clinician may support telemedicine services, palliative care, value-based care initiatives, quality improvement projects, provider education, and other strategic priorities that advance Med One's mission of delivering exceptional post-acute care.
Qualifications:- Graduate of an accredited Physician Assistant or Nurse Practitioner program (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner).
- Current national certification as a Physician Assistant or Nurse Practitioner.
- Current, unrestricted Ohio licensure as a Physician Assistant or Advanced Practice Registered Nurse (APRN).
- Current DEA registration and prescriptive authority in accordance with Ohio law, as applicable.
- Active OARRS account.
- Eligible for privileges at post-acute care facilities.
- Minimum of one year of post-acute, skilled nursing, rehabilitation, hospital medicine, geriatrics, or long-term care experience required.
- Strong clinical assessment, diagnostic, and decision-making skills.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively with physicians and interdisciplinary teams.
- Adaptability to multiple facility workflows, electronic medical record systems, and clinical environments.
- Valid driver's license with reliable transportation and willingness to travel throughout the assigned service area.
- Provide comprehensive medical care to new and established patients across multiple Med One post-acute care facilities throughout Central Ohio.
- Perform comprehensive histories, physical examinations, and interpret laboratory and diagnostic studies for medically complex post-acute patients.
- Develop, implement, and adjust individualized treatment plans in collaboration with supervising or collaborating physicians.
- Order, interpret, and communicate diagnostic and laboratory testing.
- Prescribe pharmacologic and non-pharmacologic therapies in accordance with Ohio law and scope of practice.
