Registered Nurse RN

We are seeking Registered Nurse RN to deliver patient centered care through in home visits for Medicare patients. This role primarily supports urgent and episodic interventions, working in partnership with Physicians, NPs and interdisciplinary care teams to address a range of patient needs across the care continuum.

Salary: $77,000 – $112,000 depending on experience plus mileage and full benefits package

Deliver urgent, in-home, patient care under the direction of a licensed provider throughout the assigned area including initial assessments, routine follow-up visits, and ongoing care coordination.

Conduct comprehensive home safety evaluations and document findings during patient visits.

Deliver patient care under the direction of a licensed provider by:

  • Obtaining patient histories
  • Reviewing medications and treatment plans
  • Performing physical assessments
  • Identifying and addressing health concerns observed or reported during home visits

Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures.

Maintain accurate, timely, and complete documentation

Provide patient and caregiver education

Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives.

Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning

Scheduled working hours: 10am-7pm EST (tentative)

Requirements:

Active State Licensure required; compact licensure preferred.

3+ years as an RN in a direct, patient-facing care delivery role (required).

CPR Certification

Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered.
