Registered Nurse (RN) – Triage, General Internal Medicine

• Collaborate with clinic team members to provide evidence-based, patient-centered care
• Help patients stay up to date on care plans and related health-improvement goals
• Focus on high-risk medically complex patients
• Support patients on specific disease registries
• Coordinate care for patients with disorders or chronic physical or intellectual disabilities
• Support patients during care transitions, including pediatric-to-adult care, recent inpatient admissions and discharges, emergency-department follow-up, and care-facility discharges
• Act as a liaison with patients’ family members and other agencies
• Facilitate improved communication to provide comprehensive whole-person care
• Stay current on standards
• Assist with programs and processes

• Associate Degree in Nursing from a CCNE- or ACEN-accredited nursing program
• Current Ohio Registered Nurse license
• CPR/BLS certification
• Knowledge of triage procedures
• Clinic, physician office, and/or urgent care experience
• Ability to maintain quality, safety, and/or infection control standards
• Knowledge of procedures and standards for administering medications and patient care aids
• Ability to educate and clearly communicate with patients and families about disease, care, and treatment
• Ability to convey information in writing and verbally
• High interpersonal skills for sensitive and confidential situations
• Poise, tact, and diplomacy
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions

Demonstrates expertise in patient-centered care, focusing on high-risk medically complex patients while ensuring effective communication with patients and families. Proficient in coordinating care transitions and maintaining quality and safety standards in clinical settings.
