Collaborate with clinic team members to provide evidence-based, patient-centered care.
Help patients stay up to date on their care plans and related health-improvement goals.
Focus on high-risk medically complex patients, patients on specific disease registries, patients with disorders or chronic physical or intellectual disabilities, and patients in care transitions.
Act as a liaison with patients’ family members and other agencies.
Facilitate improved communication to provide comprehensive whole-person care.
Stay up to date on standards and assist with programs and processes.

Requirements
  • Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program.
  • Current Ohio Registered Nurse license.
  • CPR/BLS certified.
  • Knowledge of triage procedures along with clinic, physician office and/or urgent care experience.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of appropriate procedures and standards for the administration of medications and patient care aids.
  • Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
  • Ability to convey information both in writing and verbally.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Position continually requires demonstrated poise, tact and diplomacy.
  • Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions.
