Center Nurse: Chronic Care & Patient Education

Oak Street Health is seeking a full-time Registered Nurse to join our Columbus on-site clinic. The Center Nurse will work within a value-based care model, performing assessments, triage, wound care, vaccines and medication administration while coordinating with the care team to reduce hospitalizations.

You will educate patients, conduct point-of-care testing, review labs, and support transitions from hospital to clinic with protocol-driven chronic condition management.
