Job Description
A nonprofit client of Insight Global is seeking Case Managers to provide individualized support, guidance, and coordination of services for participants enrolled in personal development and reentry-focused programs. This role helps participants identify goals, overcome barriers, and access resources through structured coaching and case management. The Coach collaborates with program and clinical staff to promote participant stability, engagement, and positive outcomes.
ParticipantSupport & Coaching
- Build supportive, goal-focused relationships with assigned participants
- Conduct assessments to identify participant needs, strengths, and barriers
- Support participants in developing and working toward individualized service or action plans
- Provide ongoing coaching, motivation, and accountability
- Maintain an active caseload and track participant progress
- Coordinate referrals to internal programs and external community resources
- Advocate for participants in education, employment, behavioral health, and reentry systems
- Monitor service utilization and follow up on referrals
- Maintain accurate and timely case notes, service plans, and progress reports
- Ensure documentation meets program and funder requirements
- Participate in data collection and reporting activities
- Collaborate with program coordinators, clinical supervisors, mentors, and other staff
- Participate in case reviews, supervision, and team meetings
- Communicate participant progress, challenges, and concerns appropriately
- Adhere to organizational policies, procedures, and ethical standards
- Maintain confidentiality and professional boundaries
- Participate in required trainings and professional development
- Perform other duties as assigned by program leadership

- High School Diploma or GED required;
Associate’s or Bachelor’s degree in Human Services, Social Work, or a related field preferred - 2+ years of experience as a case manager in a social work/community-based organization, or social services organization required (Experience with children and adults)
- Strong interpersonal, communication, and organizational skills
- Ability to engage participants using a strengths-based, goal-oriented approach
- Ability to manage multiple cases while maintaining accurate documentation
- Knowledge of community resources and referral systems preferred
