The Case Manager is responsible for coordinating, monitoring, and managing services for assigned clients or cases to ensure timely, effective, and high-quality outcomes. This role involves assessing needs, developing care or service plans, collaborating with internal teams and external stakeholders, and ensuring compliance with applicable policies and regulations.
Key Responsibilities- Assess client or case needs through interviews, documentation review, and data analysis
- Develop, implement, and monitor individualized case plans
- Coordinate services and resources across internal departments and external partners
- Serve as the primary point of contact for assigned cases
- Track case progress and update records accurately and timely
- Identify risks, barriers, or gaps in service delivery and recommend solutions
- Proven experience in case management, care coordination, or a related role
- Strong organizational and time-management skills
- Excellent communication and interpersonal abilities
- Ability to manage multiple cases simultaneously
- Proficiency in case management systems or standard office software
