* Job Summary:

** The Court Case Manager provides comprehensive case management and resource linkage for patients of Basecamp Recovery Center who are involved in the criminal justice system. This position supports patients in addressing substance use, mental health, legal, and social service needs, serving as a liaison with courts, probation officers, parole authorities, and other stakeholders to promote successful recovery and reintegration from incarceration.

Court Case Managers advocate for appropriate treatment interventions determined by the clinical treatment team, and provide ongoing support to resolve legal challenges.

Job Duties /Responsibilities:

Uphold the Mission Statement of Basecamp Recovery Center in all actions, performance, and decision-making.

Participate in all required staff meetings, training sessions/activities, weekly clinical supervision sessions, and retreats.

Maintain professional and respectful relationships with all patients, colleagues, and external partners.

Comply with all agency policies, procedures, patient rights, regulations, and applicable federal,state, and local laws, including HIPAA & confidentiality, 42 CFR Part II, etc.

Provide high-quality substance use disorder and mental health intervention services to assigned patients in accordance with all CARF, OhioMHAS, Medicaid, and other accrediting and regulatory entities.

Serve as a primary liaison between Basecamp Recovery Center, court systems, probation/parole authorities, and other legal entities to support patient compliance with legal obligations,successful reintegration into the community post-incarceration, and successful treatment outcomes.

Develop, implement, and monitor individualized service plans in collaboration with the clinical team to address patient needs in areas such as:
* Housing stability
* Employment readiness
* Mental health treatment
* Substance use recovery
* Legal compliance and court requirements
* Transportation* Family reunification
* Conduct regular case management sessions, including progress monitoring, goal reviews, and barrier resolution as determined by patient level of need and details of the case plan.
* Accompany patients to all assigned court hearings, probation meetings, and appointments, collaborating with legal counsel as needed and requested by the patient.
* Make appropriate referrals to community services for mental health counseling, medical care,legal assistance, and other supportive resources when indicated and directed.
* Prepare and maintain accurate, timely, and confidential documentation, including case notes,
* progress reports, court updates, ROIs, and collateral contacts in accordance with agency and regulatory requirements.
* Assist with coverage of other case managers’ caseloads as needed during absences or high-volume periods, as directed by the Court Liaison/Director.
* Participate in treatment team meetings, case reviews, interventions, and quality improvement activities.
* Stay informed on emerging trends, resources, and best practices in substance use disorder treatment, case management, and criminal justice rehabilitation.
* Perform other duties as assigned by the Court Liaison/Director.## Qualifications
** Required Skills/Abilities:
** Proficiency with Google Suite, Microsoft Office and electronic health record systems (EHRs)  Strong oral and written communication skills  Sound judgment and ethical decision-making ability  Effective problem-solving and conflict resolution skills  Ability to build rapport and work collaboratively with diverse populations, including individuals  with complex legal, behavioral health, and social needs  Knowledge of community resources and the criminal justice system

* Education and Experience:

** Associate or Bachelor’s degree in counseling, Social Work, or related field strongly preferred  High School Diploma required  CDCA required; LSW, LCDC-II, LCDC-III licensure preferred  Broad knowledge of the social service system  3-5 years work with homeless or underserved populations preferred

** Hold and maintain an Ohio driver’s license with good driving record  Required to travel on company time  Furnish a reliable, personnel means of transportation to accomplish work objectives
