Social Work Outreach Coordinator
DutiesThe Social Work Outreach Coordinator will provide outreach services to engage homeless Veterans. They will cover the Housing Hub, offering same-day services and homeless program linkage to unscheduled Veterans. The coordinator will independently conduct interviews and assessments for program enrollment and to aid in the development of treatment plans. They will utilize Housing First and other evidence-based practice models to promote Veteran engagement and self-efficacy.
The coordinator will assist Veterans with housing placement, beginning with the process of obtaining a HUD-V ASH voucher from the Public Housing Authority (PHA) through the lease up process. They will advocate on behalf of Veterans and their families with landlords, PHA, community providers, and internal VA stakeholders. The coordinator will assist Veterans to form and achieve goals to improve quality of life, to include the pursuit of employment, engagement in medical and mental health care, increased social supports, access to benefits, and end of life planning.
They will provide community-based, clinical case management and care coordination services at the frequency commensurate with the Veteran's stage of case management, clinical needs, and preferences for care. The coordinator will conduct home visits, with the intent of removing barriers for access to care, supporting housing stability, and assessing social and community integration. They will provide a wide range of social work interventions and services in both individual or group settings, as consistent with VHA Directive 1110.02 Social Work Professional Practice and approved scope of practice.
The coordinator will ensure timely, accurate, and clear documentation in the Electronic Health Record, Homeless Operations Management and Evaluations System (HOMES), and other applicable databases or platforms. They will maintain a general knowledge of veterans' benefits and services related to special programs, service-connected compensation, and non-service connected pension benefits. Other duties as assigned, as related to the needs of the program.
Schedule
Monday through Friday, 8:00am-4:30pm
PayCompetitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental LeaveAfter 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care SubsidyAfter 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
