Bridges Liaison

Necco has an opportunity for a career as Bridges Liaison. The Bridges Liaison serves as the primary case management professional for emancipated and transitional-aged youth enrolled in the Bridges Program. This position partners closely with young adults, community providers, housing resources, employers, educational institutions, and internal teams to develop individualized plans that promote stability, self-sufficiency, and long-term success. The Bridges Liaison provides services in office, home, school, employment, and community settings while maintaining compliance with all program, state, and agency requirements.

Client Engagement & Case Management
  • Coordinate intake, eligibility determination, enrollment, and ongoing participation in the Bridges Program
  • Develop collaborative relationships with young adults built on trust, accountability, and person-centered planning
  • Complete comprehensive assessments and develop individualized case plans that support independent living goals
  • Coordinate housing, education, employment, healthcare, financial literacy, transportation, and other supportive services
  • Conduct required face-to-face contacts within established program standards
  • Encourage participation in independent living programming, support groups, and community resources
  • Monitor progress toward established goals and modify service plans as needed
Documentation & Compliance
  • Maintain timely, accurate, and complete documentation within the Electronic Health Record
  • Complete all required assessments, eligibility documentation, case plans, VPA documentation, tier determinations, and other program-specific documentation
  • Ensure all Bridges documentation is accurately maintained within Ohio SACWIS and other required systems
  • Prepare reports, court documentation, and other required records
  • Close cases appropriately when eligibility requirements are no longer met or services are voluntarily discontinued
  • Maintain compliance with all agency, state, licensing, and funding requirements

Crisis Intervention & Management

  • Participate in rotating on-call responsibilities
  • Respond appropriately to crisis situations utilizing sound clinical judgment and established agency protocols
  • Document critical incidents within required timelines
  • Immediately report concerns related to abuse, neglect, safety risks, suicidal ideation, housing instability, or other situations requiring intervention
  • Collaborate with supervisors and community partners to ensure youth safety
Collaboration & Community Partnerships
  • Develop and maintain productive relationships with courts, housing providers, schools, employers, behavioral health providers, and community agencies
  • Participate in court hearings and advocate for the needs of enrolled young adults
  • Work collaboratively with internal departments to coordinate services and improve outcomes
  • Support teammates and contribute to a positive, solution-focused work environment
Team Responsibilities
  • Participate in team meetings, supervision, required training, and quality improvement activities
  • Maintain current knowledge of Bridges regulations, Independent Living best practices, and community resources
  • Contribute to continuous improvement initiatives that strengthen program quality and client outcomes
  • Perform additional duties as assigned
