Case Manager
Start Date: 8/3/2026
Pay Rate: $1800.00 - $2700.00
The Coach (Case Manager) provides individualized support, guidance, and coordination of services for participants enrolled in People Like Me (PLM) programs, including Men Like Me and The Outlet. This role supports participants in identifying goals, overcoming barriers, and accessing resources through structured coaching and case management. The Coach works collaboratively with program and clinical staff to support participant stability, engagement, and positive outcomes.
POSITIONDUTIES
- Conduct assessments to identify participant needs, strengths, and barriers
- Support participants in developing and working toward individualized service or action plans
- Provide ongoing coaching, motivation, and accountability
- Maintain an active caseload and track participant progress
- Coordinate referrals to internal programs and external community resources
- Advocate for participants in education, employment, behavioral health, and reentry systems
- Monitor service utilization and follow up on referrals
- Maintain accurate and timely case notes, service plans, and progress reports
- Ensure documentation meets agency, funder, and program requirements
- Participate in data collection and reporting activities
- Collaborate with Program Coordinators, Clinical Supervisor, mentors, and other staff
- Participate in case reviews, supervision, and team meetings
- Communicate participant progress, challenges, and concerns appropriately
- Adhere to agency policies, procedures, and Code of Ethics
- Maintain confidentiality and professional boundaries
- Participate in required trainings and professional development
- Perform other duties as assigned by program leadership
- Experience in case management, coaching, reentry, youth development, or social services preferred
- Strong interpersonal, communication, and organizational skills
- Ability to engage participants using a strengths-based and goal-oriented approach
- Ability to manage multiple cases while maintaining accurate documentation
- Knowledge of community resources and referral systems preferred
Length of Assignment: 13 weeks
Shift / Hours per week: Day Shift – Hybrid Schedule
