Basic FunctionReporting to the Resettlement Director, The CHE (Cuban Haitian Entrants) Case Manager ensures compliance with PC grant requirements. The Case Manager will address barriers to safety and wellness for especially vulnerable refugees, asylees, and other ORR-eligible populations, specifically from Cuba and Haiti. The CHE Case Manager ensures clients' basic and specific needs are met so that they can overcome barriers and make progress towards goal attainment and promotion of self-sufficiency.
Responsibilities- Develop individualized self-sufficiency plans for each client enrolled in the CHE GAP Services..
- Maintains frequent contact with assigned clients including home visits.
- Assists clients in accessing and linking with appropriate external services and public benefits.
- Ensure that clients are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary.
- Maintain up-to-date documentation in client's case file to ensure services are properly documented throughout the client's service period using Extended Reach.
- Maintains and updates database with client information, assessments, resources, and referrals.
- Participates in CHE program network meetings, stakeholder meetings and work with community partners to learn about resources for clients and make referrals when needed.
- Arranges client appointments and provides schedules for clients and other individuals who may be assisting clients (e.g., case managers, interpreters, volunteers, etc.).
- Provides ongoing orientation about health-related issues, including Medicaid education, identifying, and selecting physicians who provide language appropriate services, and basic public health counseling.
- In coordination with the Community Engagement Coordinator, refer clients to one-on-one volunteer mentoring.
- Participates in public relations activities and in-service training and conferences related to the programs.
- Strives toward excellence in the areas of innovation, performance, and extraordinary acts.
- Other duties as assigned. Willing to have a flexible schedule.
- Bachelor's degree in social work or behavioral/human services field or two years of experience providing case management and/or social services preferred.
- Proficient in the use of MS Word, MS Access, MS Excel and database programs.
- Ability to work efficiently in a fast-paced environment, troubleshoot, and follow projects through with attention to detail.
- Ability to work as a team member and independently, with a high level of self-motivation.
- Exceptional interpersonal skills, including keen listening ability and relationship building.
- Must have reliable transportation, clean driving record, and valid insurance and be able to travel locally as required.
- Fluency in one of the key languages used by client populations is preferred (includes but is not limited to: Creole, French or Spanish). Proficiency in additional languages a plus.
- Cross-cultural or international experience preferred.
- Deep commitment to ETSS 'core values and ability to model those values in relationships with colleagues and partners.
