CVS Health is seeking a Field-based Case Management Coordinator to support the OhioRISE program in Northeast Ohio. You will travel to member homes and other locations up to 50% of the time, primarily in Marion, Union, Portage, Geauga, Harrison, Belmont, Monroe, Jefferson, and Carroll counties.
