Shifts Available: M-F (8am – 4:30pm)
About Solero Behavioral TransitionsSolero Behavioral Transitions in offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders.
Key Responsibilities- Provide case management services, including assessment, service planning, and ongoing support
- Utilize peer support principles to engage and motivate clients in recovery
- Assist clients in accessing community resources such as housing, employment, and healthcare
- Maintain accurate and timely documentation in compliance with agency and state guidelines
- Collaborate with clinical staff to coordinate care and support treatment goals
- Facilitate individual and group sessions as appropriate
- Advocate for clients and promote a recovery-oriented environment
- Active CDCA certification in the state of Ohio (required)
- High School Diploma or GED required
- Must be 21 years or older
- Newly qualified grads welcome
