Full Time | Days

Pay: $54,000 - $58,000/year (Based on experience)

is currently seeking a Community Case Manager for CHOICES for Victims of Domestic Violence in Columbus, Ohio. LSS CHOICES for Victims of Domestic Violence has been the sole resource for victims seeking shelter in Franklin County for more than four decades, providing a Temporary Emergency Shelter, 24-Hour Crisis Hotline, DV Counseling, Legal Advocacy, Peer Support Groups and more.

What will I do as a Community Case Manager with Lutheran Social Services?
  • Receive referrals in weekly case consultation with the YMCA.
  • Attend all meetings with CHOICES and other partners as needed and requested.
  • Assist clients with securing and preparing their transitional housing for occupancy as quickly as possible.
  • Work closely with clients to develop and implement intentional action-oriented goal plans with a focus on obtaining permanent stable housing, enhancing safety, and creating a meaningful quality of life.
  • Implement planning strategies that reflect client strengths and needs, safety of the household, client responsibilities, and action steps to help clients remove barriers and achieve their goals.
  • Provide budgeting and other planning assistance toward the goal of permanent housing retention and safety.
  • Refer clients to partner agencies and community resources that benefit their short- and long-term goals including physical and behavioral health care needs.
  • Assist with crisis intervention and de-escalation by providing and modeling professional and calm problem-solving behavior for clients, colleagues, and the community.
  • Work with clients to establish on-going support relationships with community resources, family, and friends.
  • Transfer clients to YMCA Rapid Rehousing team after barriers are removed and goals are obtained.
Requirements for a Community Case Manager with Lutheran Social Services:
  • Bachelor's degree in social work or related field preferred or equivalent experience required. LSW preferred.
  • Experience with crisis intervention, case management and advocacy.
  • Experience working as a team with diverse staff, volunteers and clients
  • Training and experience in Trauma-informed Care practices.
  • Knowledge of basic computer skills
  • Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required
