Location and TravelPreferably cover Nassau County and/or Eastern Suffolk but open to other towns as well!
Education and Experience Requirements- Current New York State License to practice Speech Pathology.
- Minimum of two years experience as a Speech Pathologist.
- Home Health experience is preferred.
- Ability to travel to patients' homes to make home visits.
- Ability to make clinical and managerial decisions within the scope of professional practice to perform speech therapy procedures competently and independently.
- Planning and provision of speech/language and dysphasia rehabilitation services to patients in their homes and to work with patients and families to provide training and/or assistive devices for speech/language and dysphasia rehabilitation.
- Reporting the patient’s initial status, significant changes, therapy plans and goals, on‑going status, and discharge plans.
- Providing the Agency with timely documentation including, but not limited to, assessments, plans, goals, visit care, summary reports, and billing/visit data.
- Instructing patients, families and team members regarding programs that enhance and/or facilitate care and follow‑up.
- Evaluating the patient’s social and architectural environment, making recommendations, and implementing them, as feasible.
- Complying with all regulatory criteria.
- Notifying the agency of visit schedule and changes.
- Providing professional leadership and clinical guidance and expertise to team members.
- Attending regular continuing education programs.
- Performing related duties, as necessary.
- Adhering to all aspects of Customer Service, exemplified by working together, showing empathy, courteous communication, and respect of others.
