Pay: $28.62 - $35.74

ResponsibilitiesSUMMARY: The Licensed Practical Nurse (LPN) provides support to patients and providers in conjunction with Registered Nurses (RN) under the supervision of the Practice.
Job Duties &
Essential Functions:
- Accurately collects clinical data on patients and demonstrates competency in reporting abnormalities to Medical Provider.
- Field patient phone calls and troubleshoot medication management issues.
- Process medication renewals and complete prior authorizations as needed.
- Documents nursing observations, interventions, and patient’s response to treatment in patient records.
- Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN).
- Maintains standard precautions in the provision of patient care.
- Demonstrates competency in answering phones, documenting clear, concise messages in EMR, providing accurate information to patients and providers, retrieval of reports.
- Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions.
- Conforms to policies and regulations governing patient rights and confidentiality of information.
- Adheres to protocol for patient message workflow.
- Perform all other related duties as assigned by management.
Required Education &
Qualifications:
- Graduate of an accredited school of Practical Nursing.
- Currently registered in New York State as a Licensed Practical Nurse (LPN).
- Must have excellent communications skills.
- Six months or more of experience working in a high volume ambulatory care setting answering phones, fielding messages via EMR.
Preferred Qualifications:
- Three years’ experience in ambulatory care setting required as a LPN.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff and medical providers.
