LPN, Per Diem, Every Friday and Saturday 11pm-7:15am About the Role

We are seeking a dedicated and compassionate LPN to join our team on a per diem basis, working every Friday and Saturday from 11pm-7:15am. As an LPN at Gurwin Jewish ~ Fay J. Lindner Residences, you will have the opportunity to provide exceptional care to our residents while enjoying a unique and luxurious assisted living community.

• Provide direct patient care to our residents, including administering medications, performing vital signs, and providing wound care

• Develop and implement individualized care plans in collaboration with the healthcare team

• Maintain accurate and detailed patient records

• Participate in interdisciplinary team meetings to ensure comprehensive care

• Collaborate with other healthcare professionals to ensure seamless transitions and quality care

• Provide education and support to residents and their families

• Maintain a safe and clean environment for residents and staff

• Current LPN licensure in the state of New York

• Prior experience in long-term care or assisted living preferred

• Strong clinical skills and knowledge of geriatric care
