Overview
The Nurse Support Program provides nurse home visitation services to families referred by Adams County Human Services. This program is designed to support families and help them stay together by providing nursing assessments, education, support and resources. The nurse utilizes family‑centered goals to promote self‑sufficiency, healthy caregiver‑child interaction, and reduce further county involvement. The nurse is responsible for providing nursing assessments and interventions, and meeting program goals.
ProfessionalPublic Health Nurse Role
The Professional Public Health Nurse (PHN) for the Nurse Support Program (NSP) is a unique, relationship‑based, public health nursing role that requires the nurse to establish and maintain a trusting therapeutic relationship with each client throughout all phases of the program. The nurse assesses the physical, emotional, social, and environmental needs of the family and provides health education, referrals, and case management.
This position fosters a relationship between nurse and client as a basis for proactive change and works with families referred to the Program by Departments of Human Services.
- Promote maintenance of health through teaching, needs assessment, case management, direct nursing assessment, and resource coordination, including prenatal and postpartum care, nutrition, mental health, parenting, child health, family planning, and life‑course development.
- Prioritize multiple work assignments, including home visits and timely completion of required nursing and programmatic documentation.
- Develop and maintain therapeutic relationships with a culturally diverse client population, utilizing reflection, motivational interviewing, and client‑centered approaches.
- Maintain a program‑specific active case load and proactively retain clients in the program.
- Ensure and adhere to professional boundaries.
- Collaborate with community agencies and service providers through referrals, communications, and case/team conferences.
- Represent Adams County as a health resource person, interpreting public health programs and policies to individuals and community groups.
- Build community relationships to support client referrals and to the NSP.
- Represent the NSP vision, mission, and model in actions and verbally to internal and external customers and colleagues.
- Participate in promoting public health and NSP services through community efforts, including serving on boards, committees, and presenting public health education.
- Exercise independent judgment in formulating and implementing nursing plans based on needs and shared health goals, delivering services through home visits and case management with sound decision‑making.
- Provide high quality, individualized, client‑centered care with accountability, integrity, and transparency.
- Assist clients in locating resources and making appropriate referrals.
- Evaluate client progress toward goals and modify interventions as needed.
- Consult and collaborate with other professionals, coordinating case management with health, behavioral health, and social service providers.
- Perform other duties as assigned.
- Complete NSP clinical documentation thoroughly and timely.
- Finish NSP assessments per required timelines with accuracy.
- Ensure accurate client care documentation.
- Participate in case‑load review to identify achievements and improvement areas.
- Engage in quality improvement efforts.
- Maintain confidentiality of information.
- Provide accurate testimony to courts, Child Protective Services, or law enforcement regarding child custody or neglect cases.
- Attend and participate in NSP reflective/clinical supervision sessions with the supervisor to facilitate individualized client planning.
- Schedule and complete three joint home visits with the NSP Nurse Supervisor each year, debriefing on visit experiences.
- Assume responsibility for ongoing learning and professional development meeting all competency requirements.
