Position: LVN/LPN - $34.47 to $49.87

Our PACE program brings customized medical care, social activities, and daily support for participants and their families under one roof.

This position is for a mission‑driven team player passionate about delivering high‑quality, integrated care to frail older adults. The RN will manage and deliver direct nursing care to PACE participants across clinic, adult day center, home, and virtual settings, serve as a critical member of the Interdisciplinary Team (IDT), and collaboratively provide high‑quality nursing care and advance participant care plans.

Responsibilities
  • Clinical Care Delivery:
    Perform tests, administer medications, deliver nursing interventions, and provide treatments as directed by the medical provider and Clinical Case Manager RN for participants in the clinic, day center, or home setting.
  • Collaboration and Communication:
    Communicate and collaborate with medical providers, Clinical Case Manager RNs, and interdisciplinary team members to support participants in achieving their health and wellness goals while operating within your scope of practice.
  • Nursing Documentation:
    Accurately perform and document nursing interventions, assessments, and treatments based on care plans and provider orders. Maintain thorough and up‑to‑date medical records in compliance with regulatory requirements.
  • Observation and Reporting:
    Observe participants for signs, symptoms, interactions, and progress, and promptly report findings to the medical provider and Clinical Case Manager RN to support timely clinical decisions.
  • Participant and Caregiver

    Education:

    Assist in educating participants, family members, and caregivers on health management, treatments, and care protocols according to established care plans and guidelines.
  • Interdisciplinary Team

    Collaboration:

    Work as part of an interdisciplinary team to ensure holistic care is delivered to each participant, including participation in care conferences and case discussions.
  • Other duties as assigned.
Qualifications and Experience
  • Active, unrestricted LVN license in California.
  • Minimum of 2 years of clinical experience, preferably with medically complex or older adult populations.
  • Demonstrated commitment to providing compassionate, high‑quality care for vulnerable seniors and frail older adults.
  • Strong communication skills, including active listening and conflict management to build trusting relationships with participants and families.
  • Ability to adapt quickly to different clinical settings, including clinic, day center, and home environments.
  • Proven experience with clinical interventions such as wound care, IV therapy, phlebotomy, and colostomy/ileostomy care.
  • Current CPR and First Aid certification or willingness to obtain within 60 days of employment. Alzheimer’s certification preferred but not required.
  • Fluency in both English and Spanish preferred to serve diverse participant populations.
  • Must provide proof of current CPR/BLS certification or obtain within 60 days of hire.
