RN - Case Management

8-hour days ), working every Tuesday to Saturday, including every other weekend.

The Case Manager Float will provide comprehensive case management support across multiple campuses. This role requires recent experience in an acute care hospital setting and involves working in both Walnut Creek and Concord locations.

Responsibilities include assessing patient needs, developing care plans, facilitating the continuum of care, and collaborating with multidisciplinary teams to ensure effective patient management.
