Registered Nurse Supervisor

Seeking Registered Nurses Case Managers to join our team of nurses who are committed to the highest standard of care, integrity and reliability. Hallway Healthcare provides 1:1 in‑home nursing care to retired Nuclear and Federal Workers who have received benefits through EEOICPA or FECA. These are workers who have retired from Department of Energy facilities such as Lawrence Livermore National Lab, Sandia National Lab, Lawrence Berkeley National Lab, Stanford Linear, etc.

We also serve Federal Workers such as U.S. Postal Workers.

Job Duties
  • Travel to patients for periodic home visits; optional out‑of‑state travel when required.
  • Supervise and oversee care and documentation of Licensed Nurses, CNAs, and HHAs.
  • Assess documentation to ensure adherence to the Plan of Care with the highest skilled nursing intervention.
  • Provide accurate and clear documentation of care tied to the POC and DOL approved diagnosis.
  • Perform monthly assessment of patient’s condition.
  • Audit the Medication Administration Record (MAR).
  • Regularly monitor patient threads on the secure messaging system.
  • Initiate renewal of care every 12 months and coordinate renewal appointments.
  • Prepare Clinical Review in support of eligibility and recertification assessment.
  • Create Plan of Care.
  • Attend recertification appointments with the patient and their primary physician.
  • Order supplies required at patients’ homes.
  • Review and update medications in the MAR.
  • Provide verbal report to new nurses, CNAs, and HHAs.
Qualifications
  • Current Registered Nurse (RN) license in California.
  • Bachelor’s degree in Nursing (BSN) preferred.
  • Minimum of 1 year of RN experience; case management, in‑home health, or occupational health experience preferred.
  • Minimum of 1 year supervisory experience.
  • Strong assessment, communication, and organizational skills.
  • Ability to work independently and manage multiple patient cases effectively.
  • Valid driver’s license and reliable transportation required.
Required Education, License and Certification
  • Graduated from an accredited school of professional nursing.
  • Current California RN License.
  • Current BLS Certificate.

Physical Requirements
  • Sufficient clarity of speech and hearing or other communication capabilities.

  • Sufficient manual dexterity to operate a personal computer, telephone, and related equipment.
  • Sufficient personal mobility and physical reflexes to safely lift, move, or maneuver items necessary for duties.
  • Proper ability to function efficiently in a general office environment and with frequent travel to a variety of field sites.
  • Ability to lift up to 50 pounds.

Employee works in an office environment sometimes with moderate noise levels and controlled temperature conditions and travels to patients’ homes where exposure to hazardous substances may occur. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.

Employee will travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature, while others may be cluttered, dirty, or uncomfortable.
