Heluna Health in Contra Costa County is seeking a CORE Outreach Specialist to conduct street outreach, engage unsheltered individuals, assess housing needs, and coordinate referrals to housing and services.
You will work with a multidisciplinary team to support outreach efforts, document in HMIS, and connect clients to permanent housing, shelter, and supportive services. Strong communication, driving, and safety-safe practices are required.
