Position Overview

The Community Support Facilitator (CSF) provides leadership, supervision, and operational oversight for Community Living Instructors supporting adults with developmental and intellectual disabilities in Supported Living Services (SLS) and Independent Living Services (ILS).

This role is responsible for ensuring quality services, staff development, participant safety, regulatory compliance, and implementation of individualized support plans. The CSF works closely with Regional Centers, families, conservators, and interdisciplinary teams to ensure participants receive person-centered services that promote independence and improve quality of life.

What You'll DoTeam Leadership & Staff Development
  • Supervise, coach, and mentor Community Living Instructors (CLIs).

  • Conduct staff training, performance coaching, and ongoing development.

  • Ensure employees understand participant-specific support plans and behavioral strategies.

  • Address performance concerns through coaching, corrective action, and documentation.

  • Maintain appropriate staffing levels and coordinate schedule coverage.

Participant Services
  • Ensure participants receive safe, compassionate, and person-centered supports.

  • Oversee implementation of Individual Program Plans (IPP), Individual Service Plans (ISP), behavioral support plans, and health care plans.

  • Monitor participant progress toward goals and ensure services are delivered according to Regional Center requirements.

  • Advocate for participant rights, choice, dignity, and community inclusion.

  • Assist participants in accessing community resources, public benefits, housing, and supportive services.

Quality Assurance & Compliance
  • Monitor documentation for accuracy, completeness, and regulatory compliance.

  • Ensure participant files remain current and audit-ready.

  • Review data collection and progress notes.

  • Submit Special Incident Reports (SIRs) and required documentation within established timelines.

  • Conduct routine quality assurance audits and corrective follow-up.

  • Ensure compliance with DDS, Regional Center, Title 17, licensing, and company policies.

Case Coordination
  • Serve as the primary contact for assigned participants, families, conservators, Regional Center representatives, and interdisciplinary teams.

  • Participate in IPP meetings, assessments, and service planning.

  • Coordinate participant services across multiple providers and agencies.

  • Maintain positive working relationships with community partners.

Operational Support
  • Respond promptly to participant and staff emergencies.

  • Provide direct participant support when necessary to ensure continuity of care.

  • Review incident trends and implement preventive strategies.

  • Assist with crisis intervention and behavioral support planning.

  • Support agency initiatives and continuous quality improvement efforts.
