Overview
A mission-driven healthcare organization is seeking a Primary Care Physician to join an interdisciplinary, value-based care program serving frail older adults. This role is ideal for physicians who prefer quality over quantity
, value team-based care, and are motivated by longitudinal, relationship-driven medicine.
Locations to choose include: Springfield, MA | Leominster, MA | Dartmouth, MA
Physicians provide comprehensive primary care to older adults who qualify for nursing-home–level care, with a focus on helping participants remain safely in their homes and communities. Care is delivered through a nationally recognized, fully integrated model that combines medical care, social services, rehabilitation, and care coordination under one team.
Responsibilities- Provide primary medical care across clinic, home, and facility settings
- Perform comprehensive geriatric assessments and longitudinal care management
- Collaborate daily with the interdisciplinary team on individualized care plans
- Coordinate specialty care and hospital transitions
- Engage patients and caregivers using a whole-person, patient-centered approach
- Maintain timely, accurate documentation in the EMR (Next Gen)
- Average daily visits: 2–5 patients
- Schedule: Monday–Friday, 8:30am–5:00pm
- 4 days onsite, 1 remote administrative day
- No after-hours call (first call managed by NP/RN team)
- Occasional home visits within ~35 miles
- MD or DO, eligible for Massachusetts licensure, or willingness to obtain
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
- DEA certification
- Experience with geriatric or medically complex patients preferred
- Newer graduates welcome, especially with geriatrics-focused training
- Strong alignment with interdisciplinary, mission-driven care
- Salary range: $275,000–$300,000 (based on experience)
