Title of Position: NUTRITION EDUCATION NAVIGATOR
Details of Position: F/T, 40 hrs/wk, $24.25/hr
Location of Position: CCE Jefferson County, 203 N. Hamilton St., Watertown
About the Position:
This position will serve as the Nutrition Education Navigator for Cornell Cooperative Extension Association of Jefferson County for the 1115 Medicaid Waiver program, and as such will provide screening, navigate referrals, and implement the nutrition portion of the 1115 Medicaid for both adults and children. This position will facilitate Medicaid 1115 waiver activities to include phone calls, home visits, virtual visits, and community educational activities.
These activities will screen Medicaid members for eligibility and referral needs. The educator will ensure participants follow through with referrals. Finally, the position will provide direct nutrition education to eligible referrals using evidence and research-based curricula. This position will also coordinate all administrative aspects of the Medicaid 1115 Waiver program, which includes securing new partnerships to gain access to referral sources, coordinating efforts with other staff, working with Healthy Alliance, working with CCE’s Finance Department with billing and reporting aspects.
This position will also assist the Nutrition Program Manager with marketing efforts to promote the new program.
Required Qualifications:
- Master’s Degree or Equivalent Education (Bachelor’s Degree and four (4) years of transferable relevant evidence-based research experience) with training and experience to provide nutrition education.
- Registered Dietician, nutrition certification, or the ability to obtain upon hire.
- Experience relevant to the role of the position.
- Experience/ability to work with a Medicaid funded program.
- Experience with and understanding of rural, low-income populations.
- Valid NYS Driver’s License and the ability to meet travel requirements associated with this position.
- Ability to work flexible hours which may include evenings and/or weekends, as appropriate.
- Must complete and obtain required nutrition certification.
