Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by the physician and in compliance with laws, regulations, and agency policies.

• Coordinates the total plan of care with other healthcare professionals, ensuring continuity by planning and exchanging information with physicians, agency staff, patients, families, and community resources. All within a Point of Care setting.
• Conducts initial and/or comprehensive nursing evaluations, determines primary care focus, develops the care plan within state guidelines, and submits accurate documentation in collaboration with physician orders.
• Performs comprehensive assessments at designated times and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by payers.

Education and Experience

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation (vehicle or public transit)
