COMMUNITY BRIDGES IS GROWING - JOIN OUR TEAM! $1,000 Sign-On Bonus. Community Bridges is seeking two part-time Day Program Direct Support Professionals (DSPs) to support individuals in Loudon and Penacook, NH.
Position OverviewDay Program Direct Support Professionals promote abilities-not disabilities-by building meaningful relationships and supporting individuals in developing skills, confidence, and community connections. DSPs focus on each individual's strengths, interests, and goals to foster inclusion and independence.
Current Openings- Less Than Part-Time – Loudon (8 hours/week)
- Less Than Part-Time – Penacook (15 hours/week)
Part-time positions may be combined to create a full-time schedule.
Qualifications- High School Diploma or GED required
- Experience in human services or related field preferred
- Reliable transportation, valid driver's license, and proof of automobile insurance required
- $1,000 Sign-On Bonus
- $500 paid at hire
- $500 paid after 6 months of employment. Sign-on bonus available to new hires only; re-hires and transfers are not eligible at this time.

Duties & Responsibilities
- Support individuals in building self-esteem and confidence through planned activities and community involvement
- Provide individualized supports, including personal care and skill development
- Assist individuals in presenting and maintaining a positive self-image
- Participate in Person-Centered Planning and support implementation of goals and objectives
- Complete required documentation, including data collection and monthly progress notes
- Maintain compliance with all required state and agency trainings (Relias, in-person trainings, and annual ISA sign-offs)
- Report safety, health, or human rights concerns immediately to a supervisor
- Ensure compliance with state regulations
- Perform additional duties as assigned
- Strong verbal and written communication skills
- Ability to meet documentation deadlines
- Ability to work independently and exercise sound judgment
- Ability to support individuals in acquiring daily living skills
- Ability to utilize community resources and social opportunities
- Strong teamwork and collaboration skills
- Basic computer competency
- Ability to represent Community Bridges professionally and positively
- Ability to maintain a positive attitude in stressful situations
Physical Requirements
- Environment:
Primarily community-based; in-office attendance required for trainings and meetings - Physical Demands:
- Ability to lift up to 50 lbs. and assist with transfers
- Frequent walking and standing; occasional stair climbing
- Ability to push wheelchairs and operate lifts (Hoyer, wheelchair, barrier-free)
- Ability to implement approved behavioral interventions as needed
- Access to a personal vehicle during all scheduled work hours
- Frequent travel with supported individuals using a personal vehicle
- Valid NH driver's license and safe driving record required
- Must meet Community Bridges insurance and liability standards
