The Care Navigator Manager is responsible for identifying, engaging, and building strengths-based relationships with participants of NHHRC’s Syringe Services Programs (SSPs). This position oversees regional care coordination efforts, supports Care Coordinators, and ensures participants are successfully linked to healthcare, behavioral health, substance use treatment, housing, and other essential services. The role combines direct participant engagement with strategic oversight of documentation, outcomes tracking, and community partnerships.
Key Responsibilities- Lead and participate in SSP outreach services in Seacoast, Manchester, and Concord regions.
- Serve as primary point of contact for Seacoast exchanges, taking an active lead role in SSP services in the Dover, Rochester, Somersworth, and Portsmouth areas; provide backup coverage in Manchester and Concord.
- Oversee preparation and distribution of safer use and wound care kits; manage regional supply coordination.
- Provide participant-centered care coordination and navigation services, supporting self‑identified goals.
- Facilitate referrals to healthcare providers, mental health services, substance use treatment programs, housing resources, Hepatitis/STI screening, and low‑barrier care.
- Design, implement, and evaluate care coordination strategies to improve participant outcomes.
- Ensure timely and accurate monthly data entry and documentation.
- Collaborate with community partners, healthcare systems, social service agencies, and public health stakeholders.
- Represent NHHRC at external meetings and statewide forums; advocate for harm reduction best practices.
- Partner with the Director of Education to facilitate harm reduction trainings and develop new training topics.
- Minimum 3 years of professional experience in public health, health education, social services, or related field.
- Minimum 1 year street outreach/field based care coordination experience.
- Knowledge of healthcare systems, substance use services, and community‑based resources.
- Experience in care coordination, case management, harm reduction, or community health preferred.
- Demonstrated commitment to working with diverse populations, including people who use drugs.
- Excellent communication, organizational, and leadership skills.
- Proficiency in Google Workspace and comfort learning new data systems.
- Valid driver’s license and ability to travel regionally within New Hampshire.
- Starting salary range: $44,000 – $51,000 annually (commensurate with experience).

This position includes field‑based outreach work, community engagement, and office‑based administrative responsibilities. The Care Navigator Manager must be comfortable working in harm reduction settings and engaging with participants in a respectful, nonjudgmental manner.
NHHRC staff members are required to work in our Concord office on Mondays and Thursdays.
Reporting StructureThis position reports to the Director of Operations.
