RN Home and Health Hospice Liaison – Home Care – VNA – Full Time

The VNA of Manchester and Southern New Hampshire provides 24/7 in‑home support and services to patients in Manchester and surrounding communities. • Increase volume of referrals by building long‑term relationships and ensuring excellent customer service.
• Make regular site visits to facilitate referral coordination.
• Identify problems and adverse referral situations; work collaboratively with referral sources and agency staff to correct them; take a lead role in customer service and/or quality improvement efforts derived from surveys or client concerns.
• Maintain comprehensive working knowledge of coverage guidelines for VNA services and provide education to staff and physicians as needed.
• Maintain comprehensive working knowledge of the VNA’s markets, including major payer groups, key referral sources and competitor market positioning.
• Implement marketing plan and propose adjustments in strategy as market forces change.
• Collaborate with hospital staff and VNA staff to transition patients to VNA Manchester and monitor readmissions.

• Education: Graduate of an accredited nursing school; education in Sales and Marketing or related field required.
• Experience: 3–5 years in sales and marketing, home‑care, or related field.
• Certification/Licensure: Current licensure in New Hampshire as a professional nurse preferred.
• Software/Hardware: Familiarity with MS Outlook, Word, Excel, and Access; experienced in setting up and maintaining a database of contacts and activities.
• Other requirements: Valid driver’s license (travel throughout southern New Hampshire), proof of automobile insurance, motor vehicle background report will be conducted.
