Position: APRN / PA-C – Geriatric Program (Family Health Center)

Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA)

The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in their home, nursing facilities and occasional outpatient clinic coverage in the Family Health Center as needed. Care is delivered in collaboration with CHMG physicians, with a focus on optimizing health, independence, and quality of life for older adults. This role requires regular travel to community-based care settings.

Primary

Responsibilities
  • Provide comprehensive primary care services to geriatric patients in home-based settings
  • Obtain detailed patient histories, perform physical examinations, and assess functional status
  • Diagnose and manage acute and chronic medical conditions within scope of practice
  • Order, interpret, and follow up on diagnostic tests
  • Prescribe and manage medications in accordance with clinical guidelines and organizational standards
  • Deliver patient and caregiver education focused on preventive care, chronic disease management, and safety in the home
  • Provide medical care to a limited number of patients in nursing facilities to ensure continuity of care
  • Support physician coverage in nursing facilities as needed
  • Provide occasional outpatient clinic coverage in the Family Health Center based on operational needs
  • Collaborate closely with CHMG physicians, nursing staff, Family Medicine Residents, and interdisciplinary team members
  • Support and assist resident physicians in the care of geriatric patients
  • Maintain accurate, timely, and thorough documentation in the electronic medical record
  • Comply with all regulatory, licensing, and organizational policies and procedures
Education
  • Master's degree in Advanced Practice Nursing or completion of an accredited Physician Assistant program
Certification, Registration & Licensure
  • Active/Eligible New Hampshire APRN or Physician Assistant license
  • Current DEA registration
  • National certification in specialty area preferred
Experience
  • Minimum of 2–3 years of clinical experience in geriatrics or primary care for older adults
  • Experience with home visits and/or nursing facility care strongly preferred
  • New graduates will not be considered for this role
