APRN - Pulmonary​/Critical Care Inpatient; Per Diem - Weekends

Summary

The Advanced Practitioner provides acute and chronic care services to patients in the inpatient setting, including initial encounters and dispositions, daily rounding, consultations, preventive care, and patient education under the supervision of a supervising physician.

Completion of an Advanced Practice Nursing Program.

• Current license to practice in the state of New Hampshire.
• Current DEA License.
• Certified by the American Academy of Nurse Practitioners or American Nurses Credentialing Center.
• Current ACLS certification.

• Critical Care/Inpatient experience required.

• Obtains patient health history, performs physical examination, develops a relevant differential diagnosis, and documents encounters appropriately.
• Orders, interprets, and follows up on diagnostic tests.
• Diagnoses acute and chronic medical problems within the scope of practice or formulates a comprehensive differential diagnosis and takes necessary steps to obtain a diagnosis, including appropriate specialty consultations.
• Prescribes and manages medications within the practice standards.
• Provides individualized patient education related to preventive care according to guidelines and relative to the care of the hospitalized patient.
• Documents in the medical record according to the department standards.
• Communicates effectively with the multidisciplinary members of the patient’s care team, including consultants and specialists.
• Performs medication reconciliation upon admission and discharge to ensure medication safety, eliminate interactions, and avoid omissions upon discharge.

The noise level in the work environment is usually moderate.
