Role Overview :
The Licensed Practical Nurse (LPN) provides personalized, in-home nursing care, following established care plans created by Registered Nurses. They collaborate with patients, caregivers, and healthcare team members to deliver safe, effective treatment while maintaining professional standards and scope of practice.
Key Responsibilities- Implement and document individualized care plans, including nursing interventions and patient responses.
- Educate patients and caregivers to promote understanding and independence.
- Communicate with healthcare providers regarding patient progress and changes in condition.
- Participate in team meetings, agency projects, and quality improvement activities.
- Attend in-service training and travel safely within the service area.
- Graduate of an accredited nursing program with a current New Hampshire or compact state license.
- Minimum of one year of recent medical/surgical or skilled nursing experience.
- Valid driver’s license, reliable transportation, and acceptable driving record.
