What You’ll Do- Lead hospice admissions and care transitions within a hospital setting
- Provide advanced pain and symptom management for complex patients
- Deliver Hospice House–level care when inpatient placement isn’t immediately available
- Seamlessly transition patients to home hospice care when Hospice House beds are full, ensuring continuity, comfort, and dignity no matter the setting
- Partner closely with physicians and hospital care teams as a trusted clinical expert
- Support and educate patients, families, and clinicians on hospice philosophy and goals of care
- Advocate for comfort, dignity, and deeply patient-centered decisions
- Hospital-based role — no traditional home hospice caseload
- Practice at the top of your license with real clinical autonomy

- Active RN license in New Hampshire (or eligibility)
- 2+ years of RN experience (hospice, palliative care, ICU, ED, oncology, or acute care preferred)
