As an LPN Licensed Practical Nurse, you will:

  • Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring

  • Work closely with other healthcare professionals to develop and implement individualized care plans

  • Conduct initial patient assessments and maintain accurate and up-to-date records

  • Participate in interprofessional team rounds and maintain open communication with patients, families, and colleagues

  • Uphold the highest standards of patient safety, dignity, and respect

Responsibilities
  • Provide competent and compassionate care to patients, including administering medications, wound care, and vital sign monitoring

  • Collaborate with other healthcare professionals to develop and implement individualized care plans

  • Conduct initial patient assessments and maintain accurate and up-to-date records

  • Participate in interprofessional team rounds and maintain open communication with patients, families, and colleagues

  • Uphold the highest standards of patient safety, dignity, and respect

  • Perform other duties as assigned by the nursing leadership team

Requirements
  • Current LPN Licensed Practical Nurse license in the state of Vermont

  • High school diploma or equivalent required; associate's or bachelor's degree in nursing or a related field preferred

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment

  • Strong attention to detail and ability to maintain accurate records
