Licensed Practical Nurse

Harris Hill Center, a Genesis Health Care facility, seeks a Licensed Practical Nurse to provide compassionate bedside care in our post-acute rehab and long-term care setting. The LPN administers medications and treatments, monitors vital signs, supports interdisciplinary care plans, and ensures accurate documentation in line with regulatory standards. You will educate patients and families, uphold infection control and patient safety protocols, and collaborate closely with RNs, therapists, and physicians.

Responsibilities

• Administer medications and treatments per physician orders and scope of practice
• Monitor, document, and report changes in patient condition to RN and physician
• Perform basic nursing procedures, including wound care and catheter care
• Accurately document care in EHR and maintain regulatory compliance
• Participate in care planning and interdisciplinary team meetings
• Educate patients and families on care plans, medications, and discharge needs
• Uphold infection control, safety, and fall-prevention protocols
• Support ADLs to promote resident comfort, dignity, and independence
• Assist with admissions, transfers, and discharges
• Collaborate with nursing assistants, RNs, and rehab staff to optimize outcomes
