Harris Hill Center, a Genesis Health Care skilled nursing and rehabilitation facility in Concord, NH, is seeking a Licensed Nursing Assistant to join our resident-focused care team. In this role, you will provide hands-on support with daily living activities, mobility, hygiene, and comfort under the direction of licensed nurses. You will monitor and report changes in condition, assist with restorative and rehab services, and maintain accurate documentation.

Responsibilities

  • Provide direct resident care including bathing, grooming, toileting, and mobility assistance under RN/LPN supervision
  • Measure and record vital signs, intake/output, and report changes in resident condition promptly
  • Support residents with feeding, hydration, and comfort needs while promoting dignity and independence
  • Assist with restorative and rehabilitation activities as directed by the clinical team
  • Maintain accurate, timely documentation in accordance with facility and regulatory standards
  • Follow infection control, safety, and HIPAA procedures at all times
  • Communicate effectively with nurses, therapists, residents, and families to support care plans
  • Assist with admissions, transfers, and discharges as needed
  • Help maintain a clean, safe, and organized resident environment
  • Participate in training, in-services, and quality improvement initiatives
