Advocate Health is seeking a Home Health Liaison RN in New Hampshire to collaborate with hospital and clinic teams and educate patients about home care, palliative and hospice options. Onsite Monday–Friday 8:00–4:30 with weekend rotation and one holiday annually, remote weekends/holidays policy noted.
The ideal candidate has an active RN license, an Associate's Degree in Nursing, and at least 3 years of clinical nursing experience including discharge planning and care coordination.
