Overview:
Our facility near Concord is seeking a Licensed Practical Nurse (LPN) to provide medical monitoring and care for clients undergoing withdrawal management. New graduates and experienced LPNs are both encouraged to apply.

• Schedule:
  
  Monday, Tuesday, Friday, and Saturday
• Competitive pay: $38–46 per hour (DOE)

• Perform intake assessments, including gathering health histories and evaluating current symptoms and conditions
• Conduct COWS (Clinical Opiate Withdrawal Scale) and CIWA (Clinical Institute Withdrawal Assessment) withdrawal assessments
• Provide medical monitoring for clients undergoing withdrawal
• Administer medications safely and accurately, following facility protocols and provider orders
• Monitor and record vital signs, promptly reporting changes in patient status
• Support development of individualized care plans in collaboration with RNs and other healthcare professionals
• Document all patient interactions and interventions accurately in the EHR
• Collaborate with the interdisciplinary healthcare team, including participation in care conferences

• Active, current Licensed Practical Nurse (LPN) licensure in New Hampshire
• Comfortable performing withdrawal assessments and medical monitoring
• Strong communication and patient education skills
