PRN, 8 hour day shift (schedule is as needed). 2 shifts a month so 24 hours a month min, 1 shift a pay period.

Case Management/Social Worker

• Conduct comprehensive psychosocial assessments for patients and families.
• Develop and implement individualized discharge plans to ensure safe and appropriate transitions of care.
• Coordinate patient care across interdisciplinary teams (physicians, nurses, therapists, etc.).
• Facilitate communication between patients, families, and healthcare providers.
• Identify and address barriers to discharge, including financial, social, and emotional issues.
• Connect patients and families with community resources, home health, rehabilitation, or long-term care facilities.
• Advocate for patients’ rights and access to necessary services and supports.
• Provide crisis intervention and emotional support for patients and families.
• Document assessments, plans, and progress in the electronic medical record (EMR) in compliance with hospital and regulatory standards.

• Master’s degree, Social Work required.
• Experience with:
  • Behavioral Health, homelessness, substance abuse, elderly care, Medicaid, assisted living, in facility placement is preferred.
  • Must be familiar with working with physicians and nurse managers.
