Make a meaningful impact every day as a Center Well Home Health Nurse. You will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will develop and manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager.

As a Home Health Registered Nurse, you will:

• Provide admission, case management and follow‑up skilled nursing visits for home health patients, as directed by the Physician and Clinical Manager
• Coordinate appropriate care, encompassing necessary healthcare personnel (such as Physical Therapists, Occupational Therapists, Licensed Practical Nurses, Home Health Aides and external providers)
• Report patient care, condition and progress to patient's physician and the Clinical Manager
• Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re‑certifications, progress updates, and care plan changes
• Prepare visit or shift reports, updates and summaries of patient records, and confer with other health care disciplines in providing optimum patient care

Required Qualifications

• Registered Nurse, licensed in the state of North Carolina, without restrictions
• A valid driver’s license, auto insurance, and reliable transportation
• Current CPR certification

Preferred Qualifications:

• Home Health, Med Surg, ICU, ER, or Acute Care experience

Pay Range

$45.00 - $63.00 - pay per visit/unit

$70,500 - $96,900 per year base pay

Additional Information

Driving Statement

You will be a part of Humana's driver safety program and therefore will need a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

TB Statement

You will be patient facing, therefore apart of Humana's Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$77,200 - $106,200 per year
