Overview
Job Title: Case Manager Pathway of Hope

Location: Concord, NC
Provides crisis intervention and case management services to clients in residence at the Concord Shelter and Pathway of Hope Clients. Plans, develops and controls the day-to-day operations of the shelter. Guides and assists clients in developing measurable steps to address their causes of homelessness. Identifies and advocates with community resources to assist clients in successful completion of the service plan. Utilizes student interns for assisting clients in the successful completion of the service plan.
Supervises the daily activities of all monitors. Ensures compliance of the program with other organizations Policies and Procedures.
- Provides crisis intervention service to clients as needed; conducts comprehensive assessments with clients to identify the cause of homelessness, needs and strengths.
- Develops a service plan to address issues identified in the assessment; guides and assists clients in developing concrete, measurable steps necessary for successful completion of the service plan.
- Determines individualized length of stay in the shelter based on the Service Plan; identifies community resources to support completion of the service plan; instructs and trains volunteers in proper methods and procedures; checks and approves work for accuracy, completeness, and timeliness.
- Identifies community resources that will assist clients in achieving the service plan; advocates with community resources to secure needed services.
- Evaluates and measures clients’ progress toward the goals and objectives identified in the service plan.
- Conducts training, guidance, and evaluation of student interns assigned to the shelter by the Director.
- Maintains all required documentation to meet Salvation Army, federal, and funding source guidelines.
- Adheres to the strictest professional confidentiality standards regarding clients and client information; abides by Salvation Army policies and procedures.
- Exercises good judgement and problem-solving techniques related to concerns and disputes from shelter clients.
- Cooperates with the Director in matters pertaining to client services; participates in weekly supervision; collaborates with shelter monitors and kitchen staff.
- Maintains conservation of Salvation Army resources.
- Assists the Director in preparing statistical and progress reports for leadership, advisory organizations, annual reports, United Way grants, contract agencies, and Divisional Headquarters.
- Maintains up-to-date knowledge of all program changes and requirements per Salvation Army policies.
- Assists with community services as needed.
- Assists in performing social service work for special or seasonal projects.
- Performs other related work as required.

Minimum Qualifications RequiredEducation And Experience:
Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
