Case Manager – Fatherhood Services

What You’ll DoAction for Children is seeking a dedicated and resourceful Case Manager to join our fatherhood services team.
In this role, you’ll work directly with fathers to identify barriers to responsible fatherhood, develop individualized plans, and connect participants with the resources, support, and encouragement they need to achieve their goals as parents and engaged members of their communities.
Key Responsibilities- Assess participants to identify barriers and challenges related to responsible fatherhood.
- Partner with participants to develop Individual Service Plans (ISPs), establish goals, and meet regularly to review progress.
- Connect participants with community resources across key areas, including housing and basic needs, education, legal and court navigation, child support, parenting time, and custody.
- Build and maintain strong knowledge of community resources and service providers throughout Central Ohio.
- Maintain current knowledge of employment and workforce development resources, including job training, placement services, and employer partnerships.
- Document participant contacts, activities, progress, and outcomes consistently, accurately, and in a timely manner.
- Complete intake and exit processes, including required surveys, assessments, and supporting documentation.
Please Note:
This position requires weekly travel throughout Central Ohio and flexibility to work some evenings and weekends.
- Bachelor’s degree combined with demonstrated experience in social services, case management, or a related human services role.
- Experience maintaining timely and accurate records and data for program reporting.
- Knowledge of Central Ohio community resources and social service systems.
- Demonstrated ability to build rapport and work effectively with individuals from a variety of backgrounds and cultures.
- Strong organizational, communication, and problem-solving skills.
- Proficiency with technology and Microsoft Office Suite.
- Valid driver’s license, reliable personal vehicle, and proof of current auto insurance.

The ideal candidate is comfortable engaging with male participants in a variety of settings, including correctional and incarcerated environments, and brings a genuine passion for supporting fathers as they work toward becoming the parents, partners, and community members they aspire to be.
