Description of Work

The Primary Care Capitation Population Health Program Support Specialist will play a central role in the design, implementation and oversight of the Medicaid Primary Care Capitation Pilot under the Rural Health Transformation Program. The Program Support Specialist will work closely with the RHTP internal teams, Regional Hubs, and participating practices. This includes coordinating practice engagement and technical assistance efforts, and supporting the integration of pilot activities with broader Medicaid initiatives.

Salary Grade Range:$47,703.00 - $ 83,479.00
Recruitment Range: $47,703.00 - $65,591.00

The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred:

  • Knowledge of Medicaid policy and value‑based payment.
  • Basic knowledge of accounting and fiscal concepts sufficient to understand payment model design, pilot performance monitoring, and implementation oversight.
  • Ability to exercise judgment and discretion in establishing, applying, and interpreting policies, procedures, and communications.
  • Ability to establish and maintain effective working relationships with internal teams, providers, vendors, partners, beneficiaries, advocates, and other stakeholders, along with the ability to plan, assign, and guide the work of team members or project contributors when required for program needs.
  • Ability to contribute to the development of communications, training materials, implementation tools, and stakeholder education resources.

NC Rural Health Transformation Program is a statewide program led by NCDHHS to transform rural health care delivery, improve health outcomes, and ensure sustainable access to care for nearly 3 million rural North Carolinians. The program is funded over five years by the Centers for Medicare & Medicaid Services (CMS).

The main goals of the program are to:

  • Catalyze innovative health care models for rural communities by changing the way providers work together to care for patients in rural NC.
  • Transform the rural care experience, building community‑based clinical, behavioral, and social supports close to home.
  • Create a sustainable rural health delivery system through underlying systems changes in rural workforce pipelines and care team models, and rural provider financial models.
