- Plan, coordinate, track, and monitor care for individuals to achieve BH I/DD Tailored Plan goals
- Engage individuals through assessment, care coordination, health promotion, and comprehensive transitional care to inform the ISP
- Adhere to service-definition deadlines and agency policies and procedures
- Support person-directed planning and plan development, emphasizing individual choice
- Educate individuals and all involved parties about services
- Coordinate care team meetings with individuals, families, and paid or unpaid supports
- Coordinate referrals to community resources, including behavioral health, physical health, and social determinants of health supports
- Manage transitions between clinical settings and life transitions
- Use person-centered practices, motivational interviewing, person-centered thinking, and trauma-informed care
- Communicate with individuals, providers, MCOs, and natural supports
- Educate individuals and families about budget development and modifying individual budgets
- Build collaborative relationships with community stakeholders in the assigned region
- Identify and provide emergency crisis response as necessary; participate in on-call structure when required
- Review utilization, reporting, clinical measurement data, and compliance issues to determine service appropriateness
- Maintain required and requested training
- Perform other responsibilities assigned by the supervisor
- Drive and travel as required
- Bachelor’s degree in Human Services required
- Licensure as a registered nurse (RN) OR a bachelor’s degree in a related health, psychology, sociology, social work, nursing, or relevant human services field
- Two years of experience coordinating LTSS and/or HCBS (NC Innovations/TBI Waiver), monitoring care delivery, and providing care management
- Two years of experience working directly with individuals with an I/DD or a TBI
- Meet North Carolina’s definition of a Qualified Professional per 10A-NCAC 27G .0104
- Valid driver’s license in the USA
- Qualified Professional (QP) certification/qualification through Monarch-DSM
- Registered Nurse (RN) certification/licensure through a State Board of Nursing
- Spanish-speaking preferred
- Flexibility to meet job demands Monday-Friday, 8a-5pm
- Driving and travel may be required
Demonstrates expertise in care coordination, person-centered practices, and crisis response while effectively engaging individuals and families in the development and management of tailored care plans. Proficient in utilizing assessment tools and community resources to support individuals with I/DD and TBI.
