Position: Pastoral Counselor (Hospice, Float) - HomeCare
Location: Mansfield Center

Location and Shift

34 Ledgebrook Dr, Mansfield (10179) – Monday‑Friday 8 AM‑4:30 PM, float between Eastern CT (Mansfield‑Norwich).

Position Overview

Pastoral Counselor – responsible for developing, implementing, and coordinating the Pastoral and Bereavement Program, including the Hospice Pastoral Program and Spiritual Volunteer program.

Responsibilities
  • Assess, implement, and supervise spiritual care consistent with patient/family beliefs.
  • Develop individual spiritual care plans and collaborate with local clergy.
  • Provide support and resources to spiritual counselors in the community.
  • Consult with hospice staff and patients at time of death or funeral.
  • Collaborate with the Hospice Interdisciplinary Team to assess spiritual needs.
  • Document care in the clinical record and maintain consistency with physician treatment plans.
  • Design and deliver bereavement programs and services.
  • Provide direct bereavement counseling to families.
  • Coordinate bereavement follow‑up by volunteers and team members.
  • Train and oversee volunteers in grief and bereavement topics.
  • Maintain records of bereavement activities and volunteer service data.
  • Handle volunteer recruitment, orientation, training, and assessment.
  • Develop and evaluate annual program plans for Spiritual and Bereavement volunteers.
  • Serve as liaison with interdisciplinary team, community partners, and external meetings.
Additional Duties
  • Network with community bereavement services and coordinate bereavement group events.
  • Develop bereavement library resources.
  • Adhere to quality assurance program standards.
  • Compile required service and statistical records.
  • Attend MSW facilitated hospice support group sessions.
Qualifications
  • Bachelor’s degree with training in pastoral care.
  • At least one year of pastoral care experience, including counseling the dying and the grieving.
