Lisbon Home Health – Caregivers, Homemakers, Companions, or PCA

We are seeking caring and dedicated professionals to join our home health team in Lisbon. The role involves supporting clients in community settings with various recovery needs.

• Assist clients recovering from strokes, concussions, brain injury, and related conditions
• Provide safety support and organization of daily activities
• Help with medical and therapeutic rehabilitation homework
• Manage medication reminders and medication schedules
• Offer social entertainment and support social skills development
• Assist clients in finding meaningful employment and money management
• Maintain client records and report progress to team members

• 1–2 years of home health care experience
• Valid driver’s license and access to a vehicle
• Strong time‑management and organizational skills
• Proficiency with smartphones and tablets
• Completed background check and drug testing prior to hire
• Ability to work collaboratively with interdisciplinary team members
